I would like to express my gratitude to [name removed] for her help and kindness a couple of weeks ago. I was having a really bad day with another service and wondered if the OT's could help. [name removed] took control of the situation, solved the problem temporarily and went out of her way to make myself and my husband feel so much better. I cannot thank her enough.

My answer to based on all aspects experience with your staff.

What was obvious is that the staff were very frustrated at not being able to supply the care required. My husband was discharged from hospital without any care package off any kind. I was told his discharge was demeanor unsafe itis a shame there is not more liaison between services.

Thank you for your feedback and I am sorry to hear there was a lack of liaison between services. The hospital ward when making the decision on discharge may not be party to all the facts of what is available at home. Hospice at Home will visit and identify that additional care is required. They will do everything they can to get this in place with the consent of the patient and their carers but can take a considerable amount of time to put in place.
